General Program Description

The Doctor of Physical Therapy program is an entry-level professional program. Students start in the fall semester and continue for 33 consecutive months. The students are enrolled in classes during fall, spring, and summer semesters. The program is offered at the Flagstaff campus as well as the new Phoenix Biomedical campus.

Clinical Experiences
Students first participate in clerkship experiences that occur during the first 2 years of the program. Following clerkships, the students participate in internship experiences. These consist of longer full-time clinical experiences at different types of facilities.

Faculty
There are 15 full-time faculty members. All are licensed as physical therapists. Eight of the full-time faculty holds post-professional doctoral degrees (PhD), and three who are certified clinical specialists. All faculty members are engaged in clinical practice at some level. The faculty has collectively published in numerous professional and scientific journals and has authored numerous texts and book chapters. The faculty to student ratio is 1:15.

Enrollment
Northern Arizona University has a total student enrollment of 19,900, with a diverse culture, gender, and age. The physical therapy program has a total student enrollment of 78 per class. Both in-state and out-of-state students are eligible to apply to the program.

Admission
Students must have a baccalaureate degree. The prerequisite course work includes 3 semester hours each of statistics, abnormal or developmental psychology, and general psychology; and 1-year lecture with lab each of anatomy and physiology, general chemistry, and physics. In addition, the GRE is required for further consideration. Applications are only accepted from the Physical Therapy Central Application System (PTCAS). The deadline for applications is November 1 of each year. The final stage of the application process is an on-campus interview in early March. Successful candidates are then selected from those who were interviewed and notified of their acceptance by mid-March. The academic profile of the current class is 3.55 (GPA), 152 (Quantitative), 155 (Verbal), and 4.2 (Analytic Writing).

Outcomes
The graduation rate (3-year average) for those students admitted to the program is 91.7%. The percentage of graduates who find employment within 6 months of passing the licensure examination is 100%. The percentage of graduates who pass the licensure examination for the past 3 years is 98%.

Tuition, fees and financial aid
Tuition for the 2012-2013 academic year is estimated at $8,054 per year for in-state students and $18,828 per year for out-of-state students. Additional fees are estimated to be $3,194 per year (includes books). Financial aid information is available through the Office of Financial Aid at Northern Arizona University. The Office of Financial Aid publishes an informational booklet describing various financial aid opportunities each year. Private sources through the Department of Physical Therapy are identified to the students as the sources become available.

Student life
The Flagstaff program admits 48 students each year and is located in Flagstaff, Arizona. This mountain campus is located at 7,000 feet adjacent to the San Francisco Peaks and enjoys a four-season climate and is known for its quality of life, superb beauty and wide variety of outdoor and cultural activities. Dormitories as well as off-campus housing are available on the Flagstaff campus. Intercollegiate athletic programs include a wide variety of men s and women s sports such as track and cross-country, basketball, volleyball, swimming, and football. The Phoenix program admits 30 students each year and is located in the heart of a large metropolitan city. This urban campus does not have dormitories, but off-campus housing is plentiful within commuting distance. The Phoenix campus enjoys a desert climate and is in close proximity to a wide variety of sporting, entertainment and cultural activities.

A. A person commits custodial interference if, knowing or having reason to know that the person has no legal right to do so, the person does one of the following:

1. Takes, entices or keeps from lawful custody any child, or any person who is incompetent, and who is entrusted by authority of law to the custody of another person or institution.

2. Before the entry of a court order determining custodial rights, takes, entices or withholds any child from the other parent denying that parent access to any child.

3. If the person is one of two persons who have joint legal custody of a child takes, entices or withholds from physical custody the child from the other custodian.

4. At the expiration of access rights outside this state, intentionally fails or refuses to return or impedes the return of a child to the lawful custodian.

B. If a child is born out of wedlock, the mother is the legal custodian of the child for the purposes of this section until paternity is established and custody or access is determined by a court.

C. It is a defense to a prosecution pursuant to subsection A, paragraph 2 if both of the following apply:

1. The defendant has begun the process to obtain an order of protection or files a petition for custody within a reasonable period of time and the order of protection or petition states the defendant’s belief that the child was at risk if left with the other parent.

2. The defendant is the child’s parent and has the right of custody and the defendant either:

(a) Has a good faith and reasonable belief that the taking, enticing or withholding is necessary to protect the child from immediate danger.

(b) Is a victim of domestic violence by the other parent and has a good faith and reasonable belief that the child will be in immediate danger if the child is left with the other parent.

D. A violation of this section is:

1. A class 3 felony if committed by a person other than the parent or agent of the parent or custodian or agent of the custodian.

2. Notwithstanding paragraph 3 of this subsection, a class 4 felony if the child or incompetent person is taken, enticed or kept from lawful custody out of this state by the parent or agent of the parent or custodian or the agent of the custodian.

3. A class 6 felony if committed by a parent or agent of the parent or custodian or agent of the custodian.

4. A class 1 misdemeanor if the child or incompetent person is voluntarily returned without physical injury prior to arrest or the issuance of an arrest warrant.

    Express Consent
    Colorado’s Express Consent Law requires any driver to consent to a chemical test if a police officer has reasonable grounds to believe the person is driving under the influence or their ability to operate a motor vehicle is impaired because of alcohol, drugs or both.

    Hearings
    If you have a notice of revocation issued by a police officer you have seven days to go to a driver license office to request a hearing. If the license was not surrendered at the time of the stop it MUST be surrendered to receive a temporary driving permit. Contact the hearing section at 303-205-5606.     Alcohol Course
    The court may require you to complete a series of alcohol classes as a condition of a plea bargain or a deferred sentence for an alcohol-related offense. There are also circumstances in which the court may not order you to take alcohol classes, but Colorado law will require these classes as a condition of driver license reinstatement.     If you are an out of state driver and need to reinstate with the State of Colorado and have a requirement for Level II Drug/Alcohol Education/Therapy you must have an evaluation done by an alcohol treatment center or alcohol therapist in the state where you reside. You cannot reinstate with the State of Colorado until you can provide documentation that you have met the requirements of the center or therapist. A summary of the evaluation should be submitted on letterhead from the alcohol treatment center or therapist and must specify the beginning and ending dates as well as the hours required to fulfill their requirements.
