What is Palliative Care?

What is palliative care?

Palliative care is care that helps people live their life as fully and as comfortably as possible when living with a life-limiting or terminal illness.

Palliative care identifies and treats symptoms which may be physical, emotional, spiritual or social. Because palliative care is based on individual needs, the services offered will differ but may include:

  • Relief of pain and other symptoms e.g. vomiting, shortness of breath
  • Resources such as equipment needed to aid care at home
  • Assistance for families to come together to talk about sensitive issues
  • Links to other services such as home help and financial support
  • Support for people to meet cultural obligations
  • Support for emotional, social and spiritual concerns
  • Counselling and grief support
  • Referrals to respite care services

Palliative care is a family-centred model of care, meaning that family and carers can receive practical and emotional support.

Who is palliative care for?

Palliative care is for people of any age who have been told that they have a serious illness that cannot be cured. Palliative care assists people with illnesses such as cancer, motor neurone disease and end-stage kidney or lung disease to manage symptoms and improve quality of life.

For some people, palliative care may be beneficial from the time of diagnosis with a serious life-limiting illness. Palliative care can be given alongside treatments given by other doctors.

Who is in the palliative care team?

Your GP, aged care worker and any other health care provider plays an important role in providing palliative care, as do family carers. They are supported by specialist palliative care services if symptoms become difficult to manage.

Where is palliative care provided?

Palliative care is provided where the person and their family wants, where possible. This may include:

  • At home
  • In hospital
  • In a hospice
  • In a residential aged care facility

Many people indicate a preference to die at home and making this possible often depends on several factors, including:

  • the nature of the illness and amount of care the person needs
  • how much support is available from the person’s family and community
  • whether the person has someone at home who can provide physical care and support for them.

How do I get palliative care?

For help and information ask your:

  • Doctor
  • Aboriginal Health Worker
  • Hospital Liaison Officer
  • HACC Worker
  • Nurse

Where can I find more information?

Thanks to medical advances and healthier lifestyles, more people are living longer. However, with age can come several chronic health conditions such as arthritis, stroke, heart disease and memory loss. That’s when you and your family turn to long term care – help with a chronic disability that impairs your capacity to preform basic activities of daily living.

Long term care can be expensive. So many families find that their retirement nest eggs turn from supporting their retirement dreams to paying for long term care. There are options to help pay for long-term care. Long-term Care Insurance pays for care at home, in an assisted living facility or in a nursing home when you can no longer care for yourself. Remember, Medicare does not pay for long-term care and, to qualify for Medicare benefits, you can only have limited financial resources. So you – or your family or children – will likely be paying for long-term care assistance.

Long-term Care Insurance isn’t right for everyone. You owe it to yourself and your family to find out if it is the right way to protect Your assets from the high costs of long-term care.

Listening to you and understanding your financial situation and potential need for LTC insurance is where we start every discussion.

  • Is having a choice of care services important to you?
  • Are you worried about being a burden to your children?
  • Do you have financial resources to pay for some care on your own?
  • Are you healthy now so you can qualify for LTC insurance?

Once we have a good understanding of your needs, we’ll shop and compare plans among a choice of leading national long term care insurance companies. You’ll be able to easily compare plans and prices to find the right solution for you.

We will be there every step of the way – from helping you explore your choices to comparing the pros and cons of plans based on your needs.

We will use our years of experience in helping thousands of people like you find the right long term care insurance plan at an affordable price.

Step 1: Educate

Being informed and educated about Long Term Care basics and options is very important. Your Longevity Alliance Long Term Care plan advisor will provide information about Long Term Care Insurance plans. Tell them about your Long Term Care needs, any financial considerations and what’s most important for you in a Long Term Care plan.

Step 2: Compare

You’ll be provided with a choice of plans and quotes. Your advisor will answer your questions and help guide you to the most appropriate plan.

Step 3: Enroll

When Can You Claim a Tax Deduction for Health Insurance?

Each year, June 28 marks National Insurance Awareness Day, serving as a reminder that it’s a good time to review your insurance coverage and ensure that it still fits your needs.

This is particularly true for health insurance and making sure you have the right amount of coverage at an affordable cost.

Health insurance can be expensive, so being able to claim a tax deduction for some of your insurance costs can help you save come tax time.

Since there are specific rules and qualifications you must follow, here’s an overview of when you can and cannot claim a tax deduction on your health insurance.

When health insurance is not tax-deductible

If you didn’t pay for health insurance, you can’t take a tax deduction for it. If your employer pays your health insurance premiums, you can’t deduct those costs. However, if an employer only pays for part of your premiums, you still may be able to claim a tax deduction for the portion you paid.

If you received a subsidy or premium tax credit to purchase an insurance plan in the Health Insurance Marketplace through the Affordable Care Act, any advanced-payment subsidy that lowered the cost of your health insurance premiums cannot be claimed as a tax deduction. However, the money you paid out of your own pocket for your premiums might be tax deductible.

You can’t take a deduction for health insurance you paid for with pre-tax money. If you have insurance through your employer, the premiums you pay are usually taken out of your paycheck before your income taxes are calculated. Since these premiums are paid with pre-tax dollars, they’re already income-tax-free, meaning you can’t claim them as a tax deduction.

Also note, you can not deduct health insurance unless you itemize your tax deductions or you are self-employed. You don t need to know if you qualify for itemized tax deductions, TurboTax will figure it out for you.

When health insurance is tax-deductible

If you’re self-employed, your health insurance premiums may be tax deductible. If you’re self-employed and not eligible for an employer-sponsored health plan through a spouse’s job, you may be eligible to write-off your health insurance premiums on your taxes. However, you can’t write off more in health insurance premiums than you earned.

Health insurance premiums paid with your own after-tax dollars are tax deductible. For example, if you purchased health insurance on your own through a health insurance exchange or directly from an insurance company, the money you paid toward your monthly premiums can be taken as a tax deduction.

Some Medicare plans are tax deductible. This includes Medicare Part B and Part D prescription coverage.

There are limits to the amount of your health insurance you can deduct. If you are able to write-off your health insurance, there are limits to how much of your premiums you can write off.

If you’re able to claim your health insurance as a medical expense deduction. you can only deduct medical expenses that exceed 10% of your adjusted gross income (7.5% if you’re 65 or older). If you’re self-employed and claimed the self-employed health insurance deduction, you don’t have to exceed the 10% threshold because you’re writing the premiums off as an adjustment to your self-employment income rather than as a tax deduction deduction.

As with all tax laws, TurboTax is up-to-date with the latest tax law changes. If you have more questions about the Affordable Care Act and how it impacts you and your taxes, you can get answers from TurboTax Health .

Lakeridge Health reports tech problems amid global cyberattack

Lakeridge Health in Durham Region was having intermittent problems with its IT system on Friday amid news dozens of countries were hit with a huge cyberextortion attack.

Lakeridge director Paul McGary didn’t say what was causing the issues or if they were similar to those that locked up computers and held users files for ransom at a multitude of hospitals, companies and government agencies around the world.

McGary said the hospital network has a contingency plan for tech problems and it was “business as usual.”

The global attack was believed to the biggest of its kind ever recorded.

The malicious software behind the onslaught appeared to exploit a vulnerability in Microsoft Windows that was supposedly identified by the National Security Agency for its own intelligence-gathering purposes and was later leaked to the internet.

Britain s national health service fell victim, its hospitals forced to close wards and emergency rooms and turn away patients. Russia appeared to be the hardest hit, according to security experts, with the country s Interior Ministry confirming it was struck.

All told, several cybersecurity firms said they had identified the malicious software in upward of 60 countries, including the United States, though its effects in the U.S. did not appear to be widespread, at least in the initial hours.

Computers were infected with what is known as “ransomware” — software that freezes up a machine and flashes a message demanding payment to release the user s data.

Mikko Hypponen, chief research officer at the Helsinki-based cybersecurity company F-Secure, called it “the biggest ransomware outbreak in history.”

Security experts said the attack appeared to be caused by a self-replicating piece of software that enters companies and organizations when employees click on email attachments, then spreads quickly internally from computer to computer when employees share documents and other files.

Its ransom demands start at $300 and increase after two hours to $400, $500 and then $600, said Kurt Baumgartner, a security researcher at Kaspersky Lab.

Chris Wysopal of the software security firm Veracode said criminal organizations were probably behind the attack, given how quickly the malware spread.

“For so many organizations in the same day to be hit, this is unprecedented,” Wysopal said.

The security holes it exploits were disclosed several weeks ago by TheShadowBrokers, a mysterious group that has published what it says are hacking tools used by the NSA as part of its intelligence-gathering.

Shortly after that disclosure, Microsoft announced that it had already issued software “patches” for those holes. But many companies and individuals haven t installed the fixes yet or are using older versions of Windows that Microsoft no longer supports and didn t fix.

By Kaspersky Lab s count, the malware struck at least 74 countries. In addition to Russia, the biggest targets appeared to be Ukraine and India, nations where it is common to find older, unpatched versions of Windows in use, according to the security firm.

Hospitals across Britain found themselves without access to their computers or phone systems. Many cancelled all routine procedures and asked patients not to come to the hospital unless it was an emergency. Doctors practices and pharmacies reported similar problems.

Patrick Ward, a 47-year-old sales director, said his heart operation, scheduled for Friday, was cancelled at St. Bartholomew s Hospital in London.

Tom Griffiths, who was at the hospital for chemotherapy, said several cancer patients had to be sent home because their records or bloodwork couldn t be accessed.

“Both staff and patients were frankly pretty appalled that somebody, whoever they are, for commercial gain or otherwise, would attack a health care organization,” he said. “It s stressful enough for someone going through recovery or treatment for cancer.”

British Prime Minister Theresa May said there was no evidence patient data had been compromised and added that the attack had not specifically targeted the National Health Service.

“It s an international attack and a number of countries and organizations have been affected,” she said.

Spain, meanwhile, took steps to protect critical infrastructure in response to the attack. Authorities said they were communicating with more than 100 energy, transportation, telecommunications and financial services providers about the attack.

Spain s Telefonica, a global broadband and telecommunications company, was among the companies hit.

Ransomware attacks are on the rise around the world. In 2016, Hollywood Presbyterian Medical Center in California said it had paid a $17,000 ransom to regain control of its computers from hackers.

Krishna Chinthapalli, a doctor at Britain s National Hospital for Neurology Neurosurgery who wrote a paper on cybersecurity for the British Medical Journal, warned that British hospitals old operating systems and confidential patient information made them an ideal target for blackmailers.

He said many NHS hospitals in Britain use Windows XP software, introduced in 2001, and as government funding for the health service has been squeezed, “IT budgets are often one of the first ones to be reduced.”

“Looking at the trends, it was going to happen,” he said. “I did not expect an attack on this scale. That was a shock.

Canada s public safety department is aware of the global attack, but in a statement it said it normally does not comment on whether it has received any reports on any specific incidents involving Canadian computers.

Lawless reported from London. Parra reported from Madrid.

Treatment Prevention

The best treatment for bipolar disorder is a combination of medication and counseling. Doctors often treat the mania symptoms with one set of drugs, and use other drugs to treat depression. Certain drugs are also used for “maintenance” — to maintain a steady mood over time.

Medical Care

  1. What Kind of Doctor Treats Bipolar Disorder? There are many medications for treating bipolar disorder, so a psychiatrist, who is best qualified to identify which drugs work best for a specific patient, should oversee treatment.
  2. Bipolar Treatment Options Bipolar disorder is treated with three main classes of medication: mood stabilizers, antipsychotics, and antidepressants.
  3. Medication Options for Bipolar Disorder Finding the right bipolar medication regimen can be challenging. For most people with bipolar disorder, two or more medications are used in combination to address the varied symptoms. Find information here about coping with side effects and tips for sticking to your treatment plan.
  4. Bipolar Medication Tips If you have bipolar disorder, you may need to take medication indefinitely and possibly for the rest of your life.
  5. Tricyclic Antidepressants Tricyclic antidepressants work by increasing activity of the brain chemicals serotonin and norepinephrine, which scientists believe play a role in brain circuits that regulate mood. However, these drugs sometimes increase the risk of mania.
  6. Antipsychotic Medications Antipsychotic medications are used as a short-term treatment for bipolar disorder to control psychotic symptoms such as hallucinations, delusions, or mania symptoms.
  7. Anticonvulsant Medications Anticonvulsants work by calming hyperactivity in the brain. They’re often prescribed for people who have rapid cycling — four or more episodes of mania and depression in a year.
  8. Calcium Channel Blockers for Bipolar Disorder Calcium channel blockers are typically used to treat high blood pressure or heart problems. Some have also been studied to treat mania or depression in bipolar disorder.
  9. Benzodiazepines for Bipolar Disorder Benzodiazepines are not a “core” treatment for mania, but they can rapidly help control certain symptoms — such as restlessness, agitation, or insomnia — until mood-stabilizing drugs can take effect.
  10. MAOIs for Bipolar Disorder Monoamine oxidase inhibitors are an extremely potent class of antidepressants that treat depression by preventing the breakdown of serotonin, dopamine, and norepinephrine in the brain.
  11. Lithium for Bipolar Disorder Lithium (Eskalith, Lithobid) is one of the most widely used and studied medications for treating bipolar disorder.
  12. Treatment for Bipolar Depression While depression episodes are far more common than manias and have a tremendous effect on the lives of patients, there are only a few established treatments for bipolar depression.
  13. Bipolar Mania Treatment If you have mania, you’ll probably need to take medicine to bring it quickly under control.
  14. Treating Bipolar Disorder in Pregnancy Few studies have been done on bipolar disorder and pregnancy, so not enough is known about the risks of untreated bipolar disorder or the risks and benefits of medications during pregnancy.
  15. Therapy for Bipolar Disorder Psychotherapy, or “talk” therapy, is an important part of treatment for bipolar disorder.
  16. Electroconvulsive Therapy and Bipolar Disorder Electroconvulsive therapy, also known as ECT or electroshock therapy, is a short-term treatment for severe manic or depressive episodes.
  • Avoiding Complications

    1. Bipolar Disorder and Suicide Prevention If you have bipolar disorder, you may have a higher chance of committing suicide. Watch out for the warning signs and reach out to your family, friends, or health professionals for help.
    2. Bipolar Suicide Warning Signs People often talk about suicide before they attempt it, so pay close attention to what they are saying and take them seriously.
    3. Self-Harm and Bipolar Disorder Self-injury, often including cutting, self-mutilation, or self-harm, is an attempt to cope with overpowering negative emotions.

    Financing Dental Care – A Guide To Your Options #federal #consolidation #loan

    Financing Dental Care

    A Guide To Your Options

Dentistry can do so much these days to improve a person s health, appearance and self-confidence. From barely noticeable braces that straighten crooked smiles to dental implants that replace missing teeth, there is a state-of-the-art solution to virtually any dental problem. Of course, like anything that involves the time and resources of skilled professionals, highly technical and sophisticated dental treatment doesn t come inexpensively; indeed, the phrase you get what you pay for probably applies doubly to dentistry. Also, the types of treatment mentioned above, as well as many others, are often considered elective and therefore may not be covered (or only partially covered) by dental insurance. This can be the case even when a given procedure offers proven health benefits.

It s important to keep in mind that dental insurance isn t really insurance meaning, it doesn t protect you from large, unforeseen expenses. It functions more like an annual discount coupon that pays a maximum of $1,000 1,500 a year. Therefore, it can only help a little for a big-ticket item provided that a particular treatment procedure meets various criteria for coverage (decided upon by your employer and the insurance company). So if you need and want anything from limited treatment to a complete smile makeover, how do you pay for it?

Well, there is always the old fashioned way: with money saved specifically for this purpose. In addition, some dentists will discount a large fee if a patient is able to pay up front, which can be a good strategy. However, it may mean a delay in gaining the use and benefits of the treatment if you have to save for it.

In the case of orthodontic treatment, dentists, generally orthodontists in this case, traditionally will accept partial payment up front and then monthly payments to cover the balance over an extended period of time. It s an arrangement that makes sense because you, as a patient, have to visit the dental office regularly over a course of months or even a couple of years to complete the treatment. But if you re having treatment that takes only a few visits over a short period of time, your dentist is less likely to offer an extended payment plan because it could be a riskier financial proposition for the practice.

Therefore, it may be that financing (in essence borrowing) or using credit to pay for the care you need is the best option for you. And there are various ways people go about doing this; some are described below. If you are contemplating one of these methods, it s important to figure out exactly what size monthly payment will fit your budget. Equally essential is to understand thoroughly all the terms of the specific credit offer you are contemplating.

Borrowing Opportunities For The Healthcare Consumer

    General-Use Credit Cards

There s a good chance your dentist accepts payment by a credit card you already have. If you use it, you won t have to apply for a new loan. The downside is that you might tie up credit you routinely rely on for other needs; plus, credit card interest rates can be higher than other types of loans. Here s a good tip from one consumer advocate: If you want to charge dental expenses to your credit card, look for a good deal on transferring that balance to a new card. Some credit card companies offer a single-digit interest rate for the life of the transferred balance, along with a transfer fee, but always make sure to read the fine print.

    Healthcare Credit Cards

In recent years, credit cards designed specifically for charging medical/dental expenses have emerged. The largest is CareCredit , offered by GE Capital and accepted by thousands of healthcare providers nationwide. CareCredit allows you to charge as many dental treatment procedures as you want until you reach your credit limit. Typically, consumers who are approved for this card will pay an interest rate of 14.9%, though a few may qualify for a promotional period in which no interest is charged. Late monthly payments, however, can result in penalty interest rates that are significantly higher (some reaching into the high twenties) and late fees of up to $35. So it is important to be comfortable with the size of the monthly payment required.

This type of arrangement should only be entered into with the utmost care particularly for no-interest promotional plans. If the entire balance is not paid within these promotional periods, the cardholder will pay an agreed-upon double-digit annual percentage rate (APR) on interest that was accruing (but not yet charged) from the day of purchase. This means that if circumstances you didn’t foresee prevent you from paying off the balance on time, it can add a large amount to your overall debt.

    Healthcare Installment Loans

If you have a large dental or medical expense that you want to finance, an installment loan might be your best option. Springstone Patient Financing is the leading company that offers this particular type of healthcare loan. Unlike a healthcare credit card account to which you can charge many smaller procedures as long as you don t go over your credit limit, an installment loan sets a payment schedule and fixed interest rate for one larger instance of borrowing say, to have your missing or failing teeth replaced with dental implants. In that way, it s very much like taking out a car loan. And like a car loan, an installment loan can be re-financed to add in additional procedures if they are large enough (typically a $1,000 or more) while still keeping one low monthly payment.

Interest rates depend on your credit history and can go as low as 3.99% APR or as high as 17.9% APR, with the average consumer paying around a 9.9% APR. There are no penalty rates. The rate you get at the beginning does not change over the life of the loan, even if a monthly payment is late. Monthly payments are often lower too, as healthcare installment loans are available for up to 84 months.

    Equity Loans

Equity loans are secured by collateral, such as the borrower s house. Because of this, equity loans tend to have lower interest rates than so-called unsecured debt such as credit cards and installment loans. That is the main advantage of an equity loan. The risk is that the lender can take the collateral i.e. your home and sell it if you don’t repay the debt. The equity you have in your home its value minus any amount you still owe on the mortgage determines the amount you can borrow. Of course, many people choose not to reduce their home equity because preserving it is a great way to preserve capital and/or save for retirement.

It s Your Finances And Your Decision

All of these options have advantages and disadvantages, which may not be so obvious at first glance. The important thing is to take your time and make sure you ask all of the questions you have. Said one healthcare finance expert, this decision is akin to the one you are making about undergoing treatment in the first place: You wouldn t want to have a dental procedure for which you did not clearly understand the risks and benefits, or if you did not completely trust the doctor. Think of this as an extension of the trust relationship you have with your healthcare provider.

If you take it slowly and enter into a financing agreement with a clear understanding of the terms and your own financial situation, it can work out very well.

If you take it slowly and enter into a financing agreement with a clear understanding of the terms and your own financial situation, it can work out very well. And be advised, if you are rushed into an agreement you don t understand or which is beyond your means, the results can be disastrous. So before you sign on the dotted line, do your research, read and understand the fine print, ask plenty of questions, and seek professional financial advice or credit counseling if you need it. After all, the point is to get a great smile, not a bad headache!

    LASIK Complications #post #lasik #care

    #

he breakthrough of LASIK surgery will forever be known as one of the greatest advances in eye care history. People who spent most of their lives wearing glasses were suddenly able to see clearly. Others happily underwent the surgery in order to spare themselves from the aggravation of contact lenses. Anyone who complains about poor vision is now immediately asked, “Have you considered LASIK?”

It is a very popular and successful treatment for many people and we are happy to offer the service to our patients who qualify for the operation.

The first thing we do when we meet with a new patient experiencing post-LASIK complications is assure them we will examine their particular issue in great detail, using a variety of tests techniques to fully qualify their problem. Any eye ailment is terrifying for all patients and we want them to know they are in good hands.

Most of these people are experiencing symptoms such as dry eyes, glare, halos or star bursting. Patients with more severe symptoms typically see smeared images, their images have no contrast and/or are blurry or perhaps they are seeing double vision. Regardless of the severity of the symptoms, we can treat them.

Dr. Irwin Azman provides treatment for people who underwent LASIK, but are now experiencing post-surgical complications. Problems with LASIK are not rare, but not all eye care physicians have experience in this area. Dr. Azman does. In fact, doctors from all over the country have referred their patients with LASIK failure to our office.

Dr. Azman has a variety of treatment options for post-LASIK failure, including the CornealCorrection PROSE Treatment. Treatments can be as simple as glasses, contact lenses, or dry eye solutions. If those options are not appropriate, we can also offer intacs or a cross-linking procedure.

What makes us so advanced with the remedy of LASIK – an operation that itself is so new? Dr. Irwin Azman, has decades of experience treating and working on various conditions related to corneas, which is the core of LASIK surgery.

In addition to LASIK, our experience is very deep in similar areas, including kerataconus and irregular corneas.

If you are experiencing post-LASIK complications or other cornea-related issues, come visit Dr. Irwin Azman. We are the best at what we do and we only provide the best for our patients and their vision. Contact us now!

Tennessee Civil Statute of Limitations Laws

For civil cases and procedures, such as lawsuits and judgments, states impose time limits called a statute of limitations . For instance, an injured person must file a lawsuit within a certain period of time after the time of the incident that caused the injury. If you fail to do so, you automatically forfeit your right to file suit; but states also accommodate for the fact that injuries may not be discovered until much later. Criminal law has a similar set of time limits, also referred to as the statute of limitations.

In Tennessee, as in other states, these time limits vary for different kinds of civil actions. For example, plaintiffs have one year in which to file a lawsuit for personal injury, three years for lawsuits involving personal property, and six years for the collection of rent and debts.

The following is a summary of Tennessee’s civil statutes of limitations, with additional links to related resources.

    Injury to Person

Note:State laws are constantly changing. While we make every effort to make sure our State Laws section is up to date, you may also want to contact a Tennessee personal injury attorney or conduct your own legal research to verify the state law(s) you are researching.

What Is the Purpose of Statutes of Limitations?

There are a couple of key reasons why civil courts in Tennessee and elsewhere impose statutes of limitations. For one, they help ensure that claims are filed soon enough after the incident that evidence remains clear and useful (including witness testimony). Additionally, these time limits prevent potential plaintiffs from threatening lawsuits indefinitely. It’s important to keep in mind that the statute of limitations may be paused (or tolled ) if the plaintiff was a minor at the time of the incident or mentally incompetent.

Free Claim Evaluation by a Tennessee Lawyer

Tennessee’s civil time limits for filing your injury case often depends on the type of injury involved. The statute of limitations is longer for collection of rent than it is for personal injury, for example. While this can all get a bit confusing, you don’t have to figure it out alone. Let a skilled injury attorney evaluate your legal claim now at no cost to you to learn more.

    Research on Early Childhood Education

Economist Robert G. Lynch found that investment in high quality prekindergarten programs generates billions of dollars in economic and other benefits for the federal and state governments. The study, Enriching Children, Enriching the Nation. published by the nonpartisan Economic Policy Institute, shows huge benefits whether programs are universal or targeted to three- and four-year-old children from poor families.
Enriching Children, Enriching the Nation (May 2007) – EPI.

A longitudinal study of participants in a Chicago public schools program serving preK through third grade students reported that at age 24 program participants had acquired more education and were less likely to commit crimes than those who did not receive the same level of service. This study is a 19-year followup report in an ongoing study of Chicago’s Child-Parent Center program, which appears in the August 2007 issue of the Archives of Pediatrics and Adolescent Medicine, a monthly journal.

PreK Program Pays Off in Academic Development

New Mexico’s preK initiative is paying off for its four-year-old participants in greater improvement in early language, literacy, and math development, according to The Effects of the New Mexico PreK Initiative on Young Children’s School Readiness . a study released August 2007 by the National Institute for Early Education Research (NIEER) at Rutgers University. The research was conducted by Jason T. Hustedt, W. Steven Barnett, and Kwanghee Jung.

Policy Makers Should Consider Full-Day Preschool (May 2006)

A study by the National Institute for Early Education Research (NIEER) finds that the benefits of full-day preschool over half-day programs are significant and concludes that “policy makers should strongly consider implementation of full-day preschool.” The report discusses a randomized trial that compared children from low-income families in half-day and full-day public preschool programs.

Results show that children attending full-day programs did better on mathematics and literacy tests than children in a 2.5 to 3-hour public preschool program and the achievement gains continued at least until the end of first grade.

    Benefits of Preschool

The journal Developmental Psychology has published research that confirms that Oklahoma’s pre-kindergarten program is successfully helping kids prepare for school. (Oklahoma is one of the few states to offer preschool to every four year old.) While learning gains were made by children of different racial and socioeconomic backgrounds, the study found the most significant improvements among Hispanic children. Researchers made a point of noting that the preschool program was staffed by well-educated, well-trained teachers who earn regular public school salaries.

This report provides an in-depth perspective on the components of quality preschool programs; principles of learning that should be incorporated into preschool curriculum (without endorsing any particular curriculum), and appropriate use of early childhood assessments. The report includes recommendations regarding professional development, development of curricula and assessment tools, and public policies at the state and federal levels. The report is from the Committee on Early Childhood Pedagogy of the National Research Council’s Commission on Behavioral and Social Sciences and Education.

    Importance of Early Experiences

The Early Childhood Longitudinal Study, Kindergarten Class of 1998-99 (ECLS-K) is an ongoing effort by the U.S. Department of Education’s National Center for Education Statistics (NCES). The study follows a nationally representative sample of approximately 22,000 children from kindergarten through fifth grade in an effort to increase public awareness of the importance of children’s early experiences in care and education evidenced by the National Education Goal on school readiness.

    Deceptive Insurance Deals (STOLIs)

There’s an evil cousin of legitimate life settlements out there, says Jack Dolan, vice president for media relations with the American Council of Life Insurers. It’s called ‘Stranger-Originated Life Insurance,’ or ‘a STOLI arrangement,’ and it’s all about getting unsuspecting consumers to deceive the insurance industry, at great personal risk to the individuals.

    Here’s how a STOLI works:

A STOLI promoter—generally a financially savvy person—approaches you about buying a hefty life insurance policy on yourself. Working independently of any insurance company, the third-party promoter promises you a big cash payment up front, will lend you money to pay the premiums for the first two years of the policy (the normal period in which an insurance company can contest a policy), and will then promise to forgive the loan.

In exchange, you’re enticed to inflate the value of your assets on the insurance application, perhaps stating that you want to guarantee your family’s security after your death. If the scheme plays out successfully, it results in a policy with a face value that can be worth several million dollars. At the outset, you must agree to transfer ownership of your policy at the end of the two-year period to the STOLI promoter and investors, who will receive those millions upon your death.

STOLI promoters typically target well-to-do, older adults who aren’t in the best of health, says Dolan. However, these peopleare healthy enough to pass a medical exam but probably don’t have more than five or so years to live. The speculators are essentially wagering on your dying soon.

The success of a STOLI depends on a seductive approach that causes you not to examine it closely enough. But there are serious consequences if you participate in this scheme, many of which you may not see immediately. The possible ramifications include the following:

Taxes, Taxes, and More Taxes: You’ll be taxed on the money the promoter gives you up front. And you could be taxed on the amount of the premium loan the promoter forgives. Some STOLI promoters pull out all the stops to court you into setting up a policy, even inviting you on all-expenses-paid luxury trips. But there is one vacation expense you’ll have to pay: taxes.

Say Goodbye to Privacy: The sooner you die, the sooner the STOLI promoter and investors get paid. They’ll all but knock on your door to check in on how you’re doing.

No More Insurance: By inflating your assets to increase the value of your insurance policy, you’ve become over-insured and are unlikely to be able to buy any more policies.

You Could Get Caught: If your policy is found to be deceptive, the insurance company will likely rescind it, refund your premiums, and leave it at that. But it may also refer the case to state law-enforcement authorities for a fraud investigation and file an action against the STOLI promoter.

Medicare Advantage vs. Medicare Cost Plans: What’s the Difference?

In 2014, about 471,200 Americans are enrolled in Medicare Cost plans, which is far less than the 15.7 million who have Medicare Advantage plans. (iStockPhoto)

Depending on where you live, Medicare Cost plans may offer the “best of both worlds” for older adults who want the flexibility of keeping their traditional Medicare benefits while simultaneously having access to out-of-network health care providers.

“The key difference between Medicare Cost plans and other types of Medicare plans is that enrollees are not restricted to the plan’s network of providers. They can go outside the network to receive Medicare-covered services,” says David Lipschutz, senior policy attorney at the Center for Medicare Advocacy, a nonprofit in the District of Columbia.

Medicare is the federal health insurance program for people who are age 65 and older and for certain individuals with disabilities. Medicare Part A covers inpatient hospital stays, while Part B covers doctor services. Medicare Part C refers to Medicare Advantage plans offered by private companies, and Medicare Part D offers prescription drug coverage.

In some parts of the country, older adults have the option to enroll in Medicare Cost plans offered by private companies. But unlike Medicare Advantage plans, people with Medicare Cost plans maintain their traditional Medicare Part A and Part B coverage.

According to Gerald Kominski, director of the Center for Health Policy Research at the University of California–Los Angeles, Medicare Cost plans are a “hybrid” between health maintenance organizations, which restrict enrollees to a network of providers, and preferred provider organizations that have a broader network of providers.

“Medicare Cost plans are essentially an HMO with the option to opt out of the limited network and see any doctor using traditional fee-for-service Medicare,” Kominski says. “It’s like an HMO with an escape clause.”

Some Medicare Cost plans include prescription drug coverage. or individuals can purchase a stand-alone Medicare prescription drug plan. Beneficiaries who seek out-of-network care pay the Part A and Part B coinsurance and deductible. To access services, enrollees must show their original Medicare card and cost plan card. People with Medicare Cost plans can return to traditional Medicare at any time, even outside the Medicare open enrollment period.

Historically, Medicare Cost plans provided a managed care option in areas of the country that traditionally had few Medicare Advantage plans, Lipschutz explains. Medicare Cost plans pay based on the reasonable cost of delivering services, while Medicare Advantage plans accept the financial risks if costs exceed fixed payments per enrollee. With Medicare Cost plans, traditional Medicare steps in only when the enrollee goes out-of-network for care. not when costs are exceeded.

That’s why consumers deciding between Medicare Cost and Medicare Advantage plans – which are both offered by private companies – should consider whether they want access to out-of-network doctors and hospitals. People with Medicare Cost plans can see out-of-network providers because traditional Medicare kicks in at that point, and the enrollee pays the deductible for Part A and Part B. It’s one of the main reasons why Medicare Cost plans are convenient for those who travel during their retirement. Consumers with a Medicare Advantage plan must see providers in-network and pay out-of-pocket if they happen to be traveling outside of their coverage area.

In 2014, a total of 471,211 Americans in 15 states and the District of Columbia have Medicare Cost plans, according to the Kaiser Family Foundation. That’s significantly less than the 15.7 million Americans enrolled in Medicare Advantage plans.

Looking at what’s available in 2015, Medicare Cost plans account for approximately 10 percent of the roughly 44,100 health insurance plans available to those 65 and older.

Overall, Minnesota has the nation’s largest concentration of people with Medicare Cost plans. An estimated 289,000 of the state’s 836,000 Medicare beneficiaries have Medicare Cost plans, according to Jean Wood, executive director of the Minnesota Board on Aging. Minnesotans will be able to choose from 21 Medicare Cost plans in 2015.

Wood attributes the popularity of Medicare Cost plans to the large number of “snow birds” – older adults who move to a warmer climate for several months during the year. “People can leave Minnesota and their Medicare Cost plan reverts to original Medicare Part A and B, which they can use fairly easily when they are out of state,” Wood says. “They appreciate having that flexibility.”

Periodic discussions by federal officials to abolish Medicare Cost plans have concerned Minnesotans throughout the years, she says, “but our citizenry has been able to persuade Congress [to keep the plans] every time the subject has come up.”

    HEALTH COLLAGES (Art)
Give each of your children a piece of heavy paper. Set out materials such as cotton swabs, tongue depressors, adhesive bandages, cotton balls, and gauze pieces. Talk about each of the materials and how they are used. Then let your children choose materials and glue or tape them onto their papers any way they wish to create collages.

    GET-WELL CARDS (Art/Social Studies)
Invite your children to make Get-Well Cards for children in a local hospital. Hand out pieces of white or light-colored construction paper. Demonstrate how to fold a paper in half to make a card. Help your children write get-well messages inside their cards, such as Get well soon, or Hope you are feeling better, and then sign their names. Then let them decorate their cards with crayons, markers or paints. Put the finished cards in a large envelope to take or mail to the hospital.

COTTON BALL COUNTING CARD GAME (Counting/Numerals)
Select ten large index cards. Number five of the cards from 1 to 5. On the remaining five cards, glue one to five cotton balls. Mix up the order of the cards and lay them out on a table. To play the game, let your children take turns counting the cotton balls on the cards and matching them with the corresponding numeral cards. Continue the game until everyone has had a turn.

    TONGUE DEPRESSOR SHAPES GAME (Shapes)
Make a gameboard by using tongue depressors to form geometric shapes, such as a square, a triangle, a rectangle, and a diamond, on a large piece of posterboard. Trace around the tongue depressors with a fine-point marker. Set out the gameboard along with the tongue depressors. Then invite your children to take turns placing the tongue depressors on top of the outlines on the gameboard to create Tongue Depressor Shapes. If desired, continue by having the children use tongue depressors to make shapes on a table or carpet.

Growing Plumeria: Care Tips for Frangipani Tree

    Botanical Name: Plumeria rubra

Plumeria care is easy as long as you can meet its needs for warm sunshine and humidity. Vigorous and a prolific bloomer, Plumeria rubra makes a beautiful potted house plant.

Thick, woody branches carry pointed leaves that grow up to 1 ft (30 cm). Big clusters of fragrant flowers grow like bouquets on branch tips from early summer through fall.

Frangipani flowers are each made up of 5 overlapping petals. The waxy blooms are about 2 in (5 cm) wide and are available in many shades of pink, red, yellow or white — or any combination.

These flowers are among the most fragrant of any tropical flowering plant. Its essential oil is commonly used as fragrance for perfume, lotion, candles and other scented products. These tropical plants are a common sight in Hawaii, where the fragrant plumeria flowers are used to make flower leis.

This Central American native prefers bright light and moderate humidity. If you can give plumeria care what it needs, you’ll enjoy this spectacular tropical plant in your home for many years.

Let the sun shine in. Plants that don’t bloom aren’t getting enough sun. Growing plumeria require at least 5 hours of direct sun each day.

Repot young plants in spring when their roots have filled the container. Move it to a pot only one size larger. When planting plumeria, care should be taken to choose a pot with drainage holes to prevent root rot. Is it too big to repot? You can top-dress it instead by removing the top 2-3 inches of soil and replacing it with fresh soil.

    Pruning Tip

Cut at a 45° angle, just below a node (the place where a leaf or branch is attached to the stem). Use sharp pruners to avoid tearing the stems.

Prune it back. You can control its size and encourage it to branch out by regular pruning. Flower clusters grow from the branch tips, so you’ll get more flowers this way. Cutting plumeria branches back by 1/3 won’t hurt it at all — this is a vigorous plant. Prune in late winter or early spring.

Give it a winter rest. Frangipani tree will go dormant in the winter months, shedding its leaves and requiring a cool rest. Put it in a cool room — a basement or garage is ideal as long as the temperature doesn’t drop below 55°F/13°C. Water sparingly and stop fertilizing until new growth begins in spring.

Do you have private insurance? Wondering if your benefit covers an air ambulance transfer? Let our Insurance Specialists take the insurance headache out of your stressful situation.

It is not easy to verify whether your private insurance policy provides benefits for an Air Ambulance transfer. Insurance coverage on air ambulance flights is determined on a case by case basis. There are numerous factors that come into play when establishing whether or not this service is covered. You must know the right questions to ask and be able to interpret the answers in order to confirm if you truly do have coverage. Each patient s condition and policy benefits are unique. Our Insurance Specialists have the experience and can determine if your claim will be accepted or rejected.

One key factor in the insurance approval process is determining if the Air Ambulance is deemed Medically Necessary .

    What is Medical Necessity ?

The American Medical Association developed the Model Managed Care Contract. Based on this Model, it was determine that Medical Necessity is defined as follows:

    • Health care or services that a prudent physician would provide to a patient for the purposes of preventing, diagnosing, or treating an illness, injury, disease, or its symptoms in a manner that is:
      • In accordance with generally accepted standards of medical practice;
      • Clinically appropriate in terms of frequency, extent, site, and duration; and
      • Not primarily for the economic benefit of the health plans and purchasers or for the convenience of the patient, treating physician, or other health care provider.

Determining Medical Necessity, although a key factor to establish, is only one of many challenges that the family faces in getting their claim paid. Air Ambulance Worldwide has insurance specialists that will navigate the maze inside insurance companies and take the stress away from you. Most air ambulance companies will complete a form or two for you then let you navigate the maze on your own. Not at Air Ambulance Worldwide. We will make every attempt to confirm your coverage, determine the likelihood of your benefit to be paid and finally we will file, track and continue to see the entire process through to the end.

If the attending physician gives a letter of Medical Necessity to the patient or family, does that guarantee payment of the Air Ambulance flight? Even though your Doctor provides a letter of Medical Necessity, your benefit contract often leaves sole discretion of necessity to the Medical Director of the insurance company. Your doctor may say that a certain procedure or service is required but the doctor at the insurance company may say that the procedure or service is not necessary. At this point you are left with very few options. This is where our insurance specialists get to work. We will continue to pursue payment until the very end while you stay focused on your loved one; not distracted by insurance frustrations.

    LASIK again ten years later?

I am with Guy448. It is a gamble – you may have great results, you may not. I chose the life time plan, had both eyes done twice last year and they are still not right. I’ll stick with glasses when I need them.

Hi, I’m 2 1/2 weeks post surgery and I’m far sighted which is much worse than near sighted but, I’m told, it may get better. In any case, I’ve read various things about lasik enhancements and I’ve made the decision to not to do it. There are risks specifically with the cornea flap and I’m going to leave well enough alone and get along with glasses.

My vision is correctable with glasses and is currently only slightly far-sighted (+.75) so I’m just going to live with it.

How far-sighted are you? Do you need glasses for distance. Or is it that you just can’t see near due to the age?

Re: LASIK again ten years later?

the risk in a second Lasik is lower than that of the first lasik because the flap is already there.

I would think all the risks are the same. The only difference is the flap is already cut. I did ask my doctor what the time frame was for lifting the flap and he said he has done them 10 years later which means to me it never really fully heals. The risks are still the same with the flap being positioned properly, and as with any cuts there is scarring, again lifting the flap is an injury to the body. There is also the risk of infections, getting the correction correct, induced sight problems like astigmatism like I have, dry eye, and more.
This is a major operation and I look back thinking how weird it was that a person can just walk into a room and have the cornea cut and lifted without a whole lot of sterilization done.

    11-20-2012 12:20 AM by Pablo7703

Re: LASIK again ten years later?

You are absolutely right. the only problems you will not have the risk of when the flap is already created are problems related to cutting blood supply to the retina and increasing the intraocular pressure in the process of creating the flap..
I am glad you are thinking this through and taking your time before rushing into it. if only I was like you before I had mine done good luck

    Adult Orthodontics

    Overview

Orthodontics is a specialty discipline of dentistry that focuses on alignment of the teeth and jaws to improve a person’s smile and oral health. Orthodontists will diagnose, treat and prevent irregularities of the teeth with the use of braces, which are fixed dental appliances, and removable dental appliances, when the braces are removed. These devices can be used to straighten teeth, correct an irregular bite, close unsightly gaps and bring teeth and lips into proper alignment. Crooked teeth that do not fit together correctly are harder to keep clean and are at risk of being lost early because of tooth decay or periodontal disease.

    Benefits

The benefits of orthodontics include a healthier mouth, a more pleasing appearance, and teeth that are more likely to last a lifetime.

    Care

Good oral hygiene is especially important when you have braces or other orthodontic appliances. Brushing twice a day with fluoride toothpaste for two minutes, flossing daily, and scheduling ongoing dental visits are all important to help keep the gums and teeth healthy.

Patients with braces should maintain a balanced diet and limit sugary snacks between meals. Your dentist may recommend avoiding certain foods that could interfere with braces or accidentally bend the wires, such as nuts, popcorn, hard candy, ice and sticky foods, such as chewing gum, caramel or other chewy candy.

    Procedure

There are a variety of options, both fixed and removable, which help move teeth, retain muscles and affect the growth of the jaws. They work by placing gentle pressure on the teeth as they move in the jaw bone.

    Traditional Braces
The most common fixed treatment option, braces consist of brackets, elastic bands and an arch wire. Brackets are bonded to the front of the tooth and used as anchors for the braces. Arch wires are placed through the brackets and small elastic bands secure the arch wire in the bracket. Tightening the arch wire puts tension on the teeth, gradually moving them into proper position. Orthodontists adjust braces every 4-6 weeks to bring about the desired results, which may be achieved in one to three years.

Today’s braces are smaller, lighter and show far less metal than in the past. They come in bright colors for kids, and clear styles preferred by many adults.

Once braces are removed, a retainer is worn on both the upper and lower jaws (or where the braces were placed originally) for at least one year to hold the teeth into place in their new position.

Although a little discomfort is expected during orthodontic treatment, today’s braces are more comfortable than ever before. Newer orthodontic techniques and materials have been developed to move teeth in a shorter period of time.

Here’s an overview of some of the other orthodontic treatment options:

    Damon Braces
Damon braces are self-ligating appliances, meaning that instead of using elastic bands to attach the arch wire to the bracket, the brackets use a slide mechanism. This eliminates the need for elastic bands, which cause friction and can slow down treatment. With damon braces teeth can move into place more freely so treatment time is decreased. Damon braces are more aesthetically pleasing because the brackets are less noticeable than traditional metal brackets. The downside is that damon braces are usually more expensive than traditional braces. Consult with your orthodontist to find out if damon braces are a good option for you.

    Invisalign
Invisalign is a set of custom-made clear plastic aligners, which are designed to shift your teeth into the proper position. About twice a month, the patient switches to a new set of aligner trays, repeating this until their teeth are in the correct position. Invisalign is intended to deliver the same benefits as traditional braces but the aligner trays are less noticeable, offering an aesthetic advantage. The aligners are removed for eating, brushing and flossing, and may not take as long to achieve good results as braces during orthodontic treatment.

    Lingual Braces
Lingual braces, also called invisible braces, are placed on the interior side of the teeth (by the tongue and palate) and are a great option for people concerned about their appearance. Similar to traditional braces, teeth are straightened using continuous gentle pressure to help them slowly shift into the proper position. Lingual braces involve more customization and may be more expensive than traditional braces.

Although treatment plans are customized, most people wear braces from one to three years, depending on their individual needs and the position of their teeth and occlusion.

Family Loans: Does The IRS Care If I Lend My Kids Money?

You might lend your kids money from the time they are little to buy a bicycle, to get that first car, or to buy a house.

Does the IRS have anything to do with these family loans?

For small loans, the answer is no.

The IRS isn t concerned with most personal loans to your son or daughter. They don t care how often you make loans, whether you charge interest, or if you ever get paid back.

    However, there are exceptions.

    Interest-free loans

If you loan a significant amount of money to your kids, say, enough to buy a house, it s important to charge interest.

If you don t, the IRS can determine that interest you should have charged was a gift. (In addition, the borrower may be more motivated to actually pay you back if there s interest involved!)

The rate of interest on the loan must be at least as high as the minimum interest rates set by the IRS .

You don t have to worry about skipped interest being subject to gift tax rules unless the interest you should have charged, combined with other gifts to the same person, exceeds $14,000 in 2013.

    Loans that are really gifts

Some people may think they can give large amounts of money to their children and say it s a loan, thus avoiding the hassle of filing a gift tax return .

The IRS is wise to that.

The loan must be legal and enforceable, or the whole thing may be deemed a gift.

Fortunately, it s easy to make a loan legal.

Write a note that shows the loan amount, when it will be paid, the rate of interest, and any collateral or security (such as a car).

Have both parties sign the note, and keep it in a safe place.

For very large loans, or for loans attached to real estate, seek legal counsel to make sure you re covered.

    Student loans for tuition

You can make student loans to your kids by drawing up a contract like any other loan.

When they graduate and start making payments to you, you will pay tax on the interest income, and the kids can take the student loan interest deduction .

Take a bad debt deduction if your child doesn t pay you back

One of the advantages of writing up a loan contract is that if your child doesn t pay, you can take a deduction for a nonbusiness bad debt.

In addition, you don t have to pay gift tax on the amount, as you would if you had given the money outright.

To take a bad debt deduction, you must prove that you tried to collect the debt.

The debtor should make a written statement that he or she cannot pay, and include a good reason, such as unemployment.

Filing a gift tax return for a loan

If I have to file a gift tax return for a loan that the IRS determines is really a gift, will I owe gift tax?

    Probably not.

You only owe gift tax when your lifetime gifts to all persons exceeds the lifetime gift tax exclusion ($5,250,000 in 2013).

For most of us, that means we re safe.

Other family loans that are safe from tax consequences

    • You lend a child $10,000 or less, and the child does not use the money for investments, such as stocks or bonds.
    • You lend a child $100,000 or less, and the child s net investment income is not more than $1,000 for the year.
